Understanding the Risk Tiers
Avoid — documented high risk
This tier covers medications with a documented, mechanistic serotonin syndrome risk when combined with methylene blue. The common thread: these drugs increase serotonin availability (by blocking reuptake or inhibiting breakdown), and methylene blue inhibits MAO-A — the enzyme that breaks down serotonin. The combination creates excess serotonin activity. SSRIs, SNRIs, MAOIs, and the antibiotic linezolid all fall here. The FDA issued a formal safety communication on this in 2011.
Caution — moderate or less-characterized risk
These medications have serotonergic activity but the interaction risk is less clearly characterized than with SSRIs and MAOIs, or the mechanism is indirect. Tricyclic antidepressants, tramadol, dextromethorphan (in common OTC cough medications), buspirone, and serotonin-precursor supplements (5-HTP, L-tryptophan) fall here. Caution does not mean safe — it means "discuss this specifically with your doctor."

These medications have no documented serotonergic interaction with methylene blue, but the combination still warrants a conversation with your prescribing physician — particularly because methylene blue has vasopressor properties (relevant for blood pressure medications) and redox activity (relevant in some clinical contexts). This tier includes blood pressure medications, anticoagulants, statins, and most psychiatric medications without direct serotonin mechanisms.
